The infection typically occurs when the virus reactivates from a latent state in the trigeminal ganglion and travels retrograde along nerve fibers to the brain. It is not spread through casual contact during an active outbreak but rather through primary or reactivated viral shedding. Antiviral Therapy and Outcomes
Intravenous acyclovir at 10 mg/kg every eight hours for 14 to 21 days is the standard treatment, reducing mortality to approximately 20% to 30% when started early. Delayed treatment beyond 48 hours from symptom onset significantly worsens outcomes, including long-term cognitive impairment and epilepsy. Supportive care in an intensive care setting addresses intracranial pressure, seizures, and autonomic instability.
