Diagnosis relies on a detailed history linking loss of consciousness directly to hair combing, brushing, or scalp manipulation, supported by tilt-table testing and exclusion of cardiac causes such as arrhythmia or structural heart disease. Neurologists and cardiologists may use implantable loop recorders or event monitors to capture asystole or bradycardia during an episode, confirming a cardioinhibitory or mixed reflex response. Management focuses on avoidance of the triggering stimulus, counter-pressure maneuvers, and, in recurrent cases, pharmacotherapy with fludrocortisone or midodrine, or cardiac pacing for documented asystole. Patients are advised to sit or lie down during hair grooming and to avoid tight hairstyles that increase scalp traction.
